Title: Clinical Nurse Navigator I
Duration: 3-Month Contract (Potential for Extension)
Location: Costa Mesa, CA 92626 (Onsite)
Schedule: Monday – Friday, 8:00 AM – 5:00 PM
Position Overview
Hoag Memorial Hospital is seeking an experienced Clinical Nurse Navigator I to coordinate and guide patient care across the healthcare continuum for patients with complex and specialty-specific diagnoses. This role serves as a patient advocate and clinical resource, ensuring seamless care coordination from diagnosis or admission through treatment, discharge, and follow-up care.
The Nurse Navigator will collaborate closely with physicians, nurses, case managers, patients, families, and interdisciplinary teams to improve patient outcomes, enhance the care experience, and support quality improvement initiatives.
Key Responsibilities
• Coordinate patient care across the continuum from diagnosis, treatment, hospitalization, discharge, and follow-up care.
• Serve as a patient advocate, ensuring patients and families understand treatment plans, care options, and available resources.
• Educate patients, caregivers, and families regarding diagnoses, treatment regimens, disease management, and healthcare services.
• Collaborate with physicians, nurses, case managers, social workers, and interdisciplinary teams to ensure continuity of care.
• Facilitate referrals and connect patients with appropriate community resources, support services, and educational programs.
• Monitor patient progress, treatment compliance, satisfaction, and clinical outcomes.
• Identify barriers to care and proactively develop solutions to improve patient access and healthcare experiences.
• Support quality improvement initiatives through data collection, tracking, reporting, and outcome analysis.
• Participate in patient care planning, discharge planning, and care coordination activities.
• Assist with orientation, education, and mentoring of nursing staff and patient care personnel as needed.
• Maintain accurate clinical documentation and regulatory compliance.
Required Qualifications
• Active California Registered Nurse (RN) License in good standing.
• Bachelor of Science in Nursing (BSN) required.
• Minimum 5 years of nursing experience.
• Fulfills mandatory stroke education requirements per certification agency.
• Minimum 3 years of experience within a specialty area.
• One plus (1+) years of Oncology nursing experience
• Strong clinical assessment, patient education, and care coordination skills.
• Experience collaborating with interdisciplinary healthcare teams.
