Job Title: Home Health/Hospice Clinical RN
Duration: 3 Months ( higher possibility to extended or convert)
Shift: Days (M - F) 8:30 am to 5 pm

Position Overview
The Home Health Clinical Nurse I (RN) provides skilled nursing care to patients in their homes, ensuring safe, high-quality, patient-centered care. This role includes comprehensive patient assessments, medication administration, IV/infusion therapy, patient education, pain management, specimen collection, and coordination with interdisciplinary healthcare teams to develop and implement individualized care plans.

Key Responsibilities
Provide skilled nursing care to patients in a home health setting.
Perform comprehensive physical and clinical assessments.
Administer medications, IV therapy, and home infusion treatments.
Develop, implement, and evaluate individualized care plans.
Monitor patient progress and modify care plans as needed.
Provide patient and caregiver education on disease processes, medications, and self-care.
Collect specimens and monitor laboratory results.
Manage pain and ensure medication adherence.
Coordinate care with physicians and interdisciplinary healthcare teams.
Maintain accurate and timely clinical documentation.
Follow infection prevention and patient safety protocols.
Participate in discharge planning and care coordination.
Ensure compliance with evidence-based nursing practices and regulatory standards.

Required Qualifications
Active California Registered Nurse (RN) license.
Associate Degree in Nursing (ADN) or higher.
Minimum 2 years of acute clinical nursing experience.
Previous Home Health nursing experience is required.
Current BLS certification.
Experience with physical assessments, IV therapy, medication administration, and patient education.
Valid driver's license with reliable, insured transportation.
