Description

Immediate Licensed Practical Nurse (LPN) - Home Health opportunity in Portage, MI. Territory: Calhoun/Branch counties.

Pay Rate:

$38.00/hr - local candidate pay rate
Travel pay package available
Mileage Reimbursement: $.49/mile for distance covered between patient homes

Work Schedule:

9:00am - 5:00pm (Monday to Friday)
40 hours per week
No weekends
13-week contract

Job Details:

Administer skilled nursing care and education under the supervision of registered nurse for adult clients in their place of residence, coordinate with the interdisciplinary team, patient/family and provider.
Support patient recovery and ability to stay at home – enabling them to spend quality time with their loved ones and doing the activities they enjoy
Empower patients and families to reach healthcare goals by educating them on disease management
Case manage and provide clinical oversight to interdisciplinary team

Qualifications

Current LPN License
Must have at least 12 months of home health experience within the past 3 years.
Hands-on HCHB experience required
Current driver’s license
American Heart Association BLS
