We are hiring a Registered Nurse (RN) for a Home Health position providing skilled nursing care to patients in their homes in the Chicago, Lincolnwood, and surrounding areas. This position is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year!

What You’ll Do:
- Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
- Perform patient assessments, medication management, and care planning
- Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
- Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
- Work independently, but never alone
- Travel to patient homes within assigned territory

RN Qualifications
- Active Registered Nurse, RN license in the state you work
- One year of RN experience and the clinical expertise to deliver quality patient care
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
