About Our CarePartners Home Health Program:
The CarePartners Home Health program provides comprehensive, skilled medical services directly to patients in the comfort and privacy of their own homes. Serving multiple counties across Western North Carolina, our program is designed for individuals recovering from surgery, managing chronic illnesses, or rehabilitating after an acute hospital stay. Our Home Health agency delivers an integrated, physician-directed care model encompassing skilled nursing, physical therapy, occupational therapy, speech-language pathology, medical social work, and home health aide services. Utilizing advanced mobile charting and clinical technology, our field clinicians travel throughout the community to perform comprehensive assessments, wound care, medication reconciliation, and home safety training. Our team works with independent precision and real-time coordination to prevent unnecessary hospital readmissions, optimize physical recovery, and restore the patient's ultimate independence.

Job Summary and Qualifications 

Provides leadership which facilitates the delivery of professional nursing care for the comfort and well-being of patients. Utilizes the nursing process of assessment, planning, implementation, and evaluation, to provide, delegate, supervise and document care and teaching of patients and family. Implements medically prescribed interventions.

What qualifications you will need:

• Education: Associate's Degree or Bachelor of Science in Nursing
• License: Must have and maintain current licensure as registered nurse with the North Carolina Board of Nursing. eNLC (Enhanced Nurse License Compact) license may apply, licensee should confirm with NCBON  Registered Nurse must be obtained within 30 days of employment start date.
• Certification: BLS Healthcare Provider
• Experience:  One year hospital nursing experience
