Work Schedule:

100% FTE, day shift. Monday – Friday; 8:00 AM – 4:30 PM. This is a hybrid role, with occasional on-site requirements at University Hospital in Madison, WI and the Administrative Office Building in Middleton, WI. Hours may vary based on the operational needs of the department.

We are seeking a Clinical Pharmacist to:

  • Develop and launch pharmacy services with collaboration from the pharmacy manager and clinical team, with the goal of enhancing patient access, optimizing medication access, and improving patient outcomes.
  • Deliver medication therapy management services to optimize patient outcomes and ensure safe, effective, and cost-efficient drug therapy.
  • Conduct telehealth consultations with headache patients referred by primary care providers and neurology specialists.
  • Provide guidance and support for medication access and affordability programs to reduce barriers to care.

 

The ideal candidate for this role would have a Pharmacy Residency focused in ambulatory care.

Qualifications
  • Bachelor's Degree in Pharmacy or a PharmD degree Required
  • Completion of an ASHP Accredited Residency training program Preferred
  Work Experience
  • 1 year of clinical patient care and orders management experience Required
  • 3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program Preferred
  Licenses & Certifications
  • Licensed to practice pharmacy in the State of Wisconsin within 90 Days Required
  • Basic Life Support (BLS) within 90 Days Required
  • Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.) Preferred
