Description
Work Schedule:
100% FTE. Day Shift. Monday – Friday, 8:00 AM – 4:30 PM. This is a hybrid position, working mainly remote with occasional on-site requirements, including training. Hours may vary based on the operational needs of the department.

We are seeking a Clinical Pharmacist to:
- Join a new pharmacist-led clinic to deliver comprehensive medication management services
- Manage GLP-1 therapies, including patient onboarding, medication initiation, titration, and ongoing monitoring under established delegation protocols
- Provide high-quality, patient-centered care independently in a fast-paced telehealth environment
- Support the needs of a rapidly growing patient population, with opportunities to expand into additional therapeutic areas
- Deliver education and training to pharmacy residents and PharmD students through both practical (clinical) and didactic experiences

Qualifications
- Bachelor's Degree in Pharmacy or a PharmD degree Required
- Completion of an ASHP Accredited Residency training program Preferred
Work Experience
- 1 year of clinical patient care and orders management experience Required
- 3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program Preferred
Licenses & Certifications
- Licensed to practice pharmacy in the State of Wisconsin within 90 Days Required
- Basic Life Support (BLS) within 90 Days Required
- Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.) Preferred
