Description
- Monday - Friday 8am - 4:30pm, no weekends, no call
- Focus on heart failure patients with aim of reducing readmissions
- Hospital and clinic setting with IP/OP duties
- Cardiology consultations, follow-ups, and discharge coordination
- Medically complex adult patient demographic
- Several years of heart failure experience required
- ACLS and BLS certification required
- Hospital privileges required
