Description
- Monday - Friday 8am - 4:30pm, no weekends, no call
- Focus on heart failure patients to reduce readmissions
- Hospital consultations, follow-ups, and discharge coordination
- Clinic duties managing hospital and heart failure follow-ups
- ACLS and BLS certification required
- Several years of experience with heart failure required
- Medically complex patient demographic
- Both inpatient and outpatient responsibilities
