Clinical Pharmacist – Transition of Care (Lobby)
Direct Hire | Full-Time | Day Shift
Location: Terre Haute, IN
Overview
A not-for-profit acute care hospital in Terre Haute, Indiana is hiring a Clinical Pharmacist focused on Transitions of Care. This permanent, day-shift role supports safe patient discharges through medication reconciliation, patient education, and close collaboration with interdisciplinary care teams.

Schedule
• Full-time, 40 hours/week
• Day shift: 7:30 AM – 5:30 PM
• No weekends or holidays
Compensation & Incentives
• Hourly range: $57.65 – $86.48
• Annual range: ~$119,912 – ~$179,878 (DOE)
• $20,000 sign-on bonus (paid at hire with a 2-year commitment)

Key Responsibilities
• Review and reconcile discharge medication lists
• Process discharge prescriptions
• Provide patient education on discharge medications
• Collaborate with physicians, nursing, and care teams
• Maintain accurate documentation in EPIC
• Precept pharmacy students and residents as applicable
Requirements
• PharmD from an accredited program
• Licensed or eligible for Indiana licensure (must obtain within 75 days of start)
• Residency or equivalent clinical experience preferred
• Board certification preferred within first year of eligibility
• New grads considered with appropriate licensure timeline
