Job Opportunity – Registered Nurse, Case Manager – Apple Valley, CA 92307

Start Date: June 29, 2026

Location: Apple Valley, California

Schedule: Day shift, 5x8-Hour (08:00 - 16:30). Patient ratios: 1:25 weekdays, 1:45 weekends. Every other weekend rotation required. 40 hours of orientation provided.

Requirements:

• Active California RN license required by start date (pending license accepted during application process)
• Minimum 2 years of acute care case management or discharge planning experience in a hospital setting
• Prior travel nursing experience required; experienced travelers only
• Epic charting system proficiency required
• Core competencies: care coordination, discharge planning, utilization review, assessing patients, and organizing family meetings
• Experience with HMOs, IPAs, arranging DME, SNF placement, and ambulance transport
• Knowledge of community resources including foster care, substance abuse treatment, homelessness services, mental health crisis intervention, advance directives, and protective services
• Ability to work independently and manage caseloads up to 25 patients
• Willingness to work 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Two professional references covering one year within the last three years
