Job Opportunity – Medical Social Worker – Mission Viejo, CA 92691

Start Date: July 13, 2026

Location: Southern California acute care facility

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30), Sunday through Thursday schedule with every other weekend required. Caseload of 20-23 patients per day. 40-hour orientation provided.

• Work as part of an interdisciplinary care management team consisting of RNs and MSW/LCSW professionals
• Comprehensive discharge planning and case management responsibilities
• Epic charting system utilized throughout the facility
• Collaborate with medical teams, insurance case managers, and utilization management to coordinate safe patient transitions
• Opportunity to work with diverse patient populations including adults and geriatrics with various diagnoses

Requirements:

• Master of Social Work (MSW) degree required; LCSW preferred
• Minimum 1 full year of acute care inpatient hospital experience required (behavioral health or emergency department experience does not qualify)
• Previous travel healthcare experience required; first-time travelers cannot be considered
• BLS certification required
• Proficiency with Epic charting system required
• California state license or pending license accepted (must be licensed upon start date)
• Trauma Level I or II experience preferred
• Must be willing to float to other facilities within the region as needed
• Ability to complete initial patient assessments within 24 hours and manage ongoing discharge planning
