Job Opportunity – Physical Therapist, Home Health – Portland, OR 97225

Start Date: August 24, 2026

Location: West Portland, OR and surrounding areas

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30). One-on-one patient care with up to 6 patient visits per day. 30 productivity points standard. No weekend requirement. No on-call requirement.

Highlights:

• Home health setting serving adults and geriatric populations with diagnoses including dementia, Parkinson's, ALS, MS, orthopedic post-op, and post-stroke rehabilitation
• Efficient scheduling with clients grouped to minimize drive time; occasional maximum radius of approximately 60 miles per day
• Free parking and IRS standard mileage reimbursement provided
• Business casual or scrubs allowed (no specific color required)
• EPIC Remote Client for Home Health charting on laptop and Rover phone app
• 91-day assignment with 40 hours of orientation

Requirements:

• Active Oregon Physical Therapist license (pending license accepted)
• Minimum 1 year of skilled home health experience required
• BCLS certification required
• OASIS documentation experience preferred
• Experience with lifting and transfer, ordering supplies, and scheduling appointments
• Reliable personal transportation required
• COVID-19 primary series vaccination plus one booster required
• EPIC charting system experience preferred
• Two professional references covering 1 year within the last 3 years
