Job Opportunity – Physical Therapist, Home Health – Portland, OR 97213

Start Date: August 24, 2026

Location: East Portland, OR and Clark County, WA surrounding areas

Schedule: Day shift, 8-hour shifts (08:00 - 16:30), 5 days per week. Up to 6 patient visits per day with a standard of 30 productivity points. No weekend requirement. No on-call requirement.

Requirements:

• Active Physical Therapy license in both Oregon and Washington (required)
• Minimum 1 year of skilled home health experience required
• BCLS (Basic Cardiac Life Support) certification
• OASIS documentation experience required
• Proficiency with EPIC Remote Client for Home Health and Rover phone application
• Reliable personal transportation required; occasional travel radius up to 60 miles per day
• Two professional references covering 1 year within the last 3 years
