Job Opportunity – Physical Therapist, Home Health – Hood River, OR 97031

Start Date: September 7, 2026

Location: Hood River, Oregon and surrounding areas

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30). One-on-one patient visits with up to 6 patients per day. 30 productivity points standard. No weekend requirement. No on-call requirement.

Highlights:

• Home health physical therapy position serving patients in Hood River and the surrounding Columbia River Gorge region
• Treat a diverse patient population including orthopedic post-op, post-stroke, dementia, Parkinson's, ALS, and MS patients
• Efficient scheduling with grouped visits to minimize drive time; maximum radius of approximately 60 miles per day
• Free parking and IRS standard mileage reimbursement provided
• Business casual or scrubs allowed; flexible attire policy
• 13-week assignment with guaranteed hours (facility may call off up to one shift per two-week pay period)

Requirements:

• Active Physical Therapist license in both Oregon and Washington (required)
• Minimum 1 year of skilled home health experience required
• OASIS documentation experience required
• Basic Life Support (BLS) certification required
• EPIC charting experience preferred (EPIC Remote Client for Home Health and Rover phone app)
• Reliable personal transportation required
• COVID-19 primary vaccination series plus one booster required
• Two professional references covering 1 year within the last 3 years
