Job Opportunity – Occupational Therapist, Home Health – Medford, OR 97504

Start Date: September 7, 2026

Location: Medford, Oregon and surrounding areas

Schedule: Day shift, 8-hour shifts (08:00 - 16:30), 5 days per week. Up to 6 patient visits per day with 30 productivity points standard. 1:1 patient ratio. No weekend requirement. No on-call requirement.

Requirements:

• Valid Oregon Occupational Therapy license (compact license accepted; pending licenses considered)
• Minimum 1 year of skilled home health experience required
• BCLS certification required
• Oasis documentation experience required
• COVID-19 primary vaccination series plus one booster required
• Reliable personal transportation required
• Two professional references covering 1 year within the last 3 years
