Description
Job Opportunity – Physical Therapist, Home Health – Medford, OR 97504
Start Date: September 8, 2026
Location: Medford, Oregon and surrounding areas
Schedule: Day shift, 8-hour shifts (08:00 - 16:30), Monday through Friday. This position requires 5x8-hour shifts; 4x10s or 3x12s cannot be accommodated. Typical caseload includes up to 6 patient visits per day with a standard productivity expectation of 30 points. No weekend or on-call requirements.

- Efficient scheduling with grouped visits to minimize drive time; occasional travel up to 50+ miles
- EPIC and Oasis charting systems utilized

- 13-week assignment with guaranteed hours (facility may call off up to one shift per two-week pay period)
Requirements:
- Active Oregon Physical Therapist license (pending licenses accepted)
- Minimum 1 year of skilled home health experience required
- BCLS (Basic Cardiac Life Support) certification required
- Oasis documentation experience required
- Reliable personal transportation required for patient visits
- COVID-19 primary series vaccination plus one booster required
- Two professional references covering 1 year within the last 3 years
