Description
Job Opportunity – Physical Therapist, Home Health – Portland, OR 97225
Start Date: September 8, 2026
Location: West Portland, OR and surrounding areas
Schedule: Day shift, 8-hour shifts (08:00 - 16:30), Monday through Friday. Typically 1:1 patient visits with up to 6 patients per day, 30 productivity points standard. No weekend or on-call requirements.
Highlights:
- Home health setting serving adults and geriatric populations with diverse diagnoses including dementia, Parkinson's, ALS, MS, orthopedic post-op, and post-stroke rehabilitation
- Well-organized scheduling minimizes drive time; occasional travel up to 60 miles (60-90 minute total drive time) per day with free mileage reimbursement at IRS standard rates
- Charting via EPIC Remote Client for Home Health on laptop and Rover phone app; OASIS documentation required
- Business casual or scrubs allowed; flexible dress code with no specific color requirements
- 91-day assignment with guaranteed hours; facility may call off up to one shift per pay period
- Local candidates welcome; considered local within 50-75 mile radius
Requirements:
- Active Oregon Physical Therapy license required
- Minimum 1 year of skilled home health experience required
- BCLS (Basic Cardiac Life Support) certification required
- OASIS documentation experience required
- COVID-19 primary vaccination series plus one booster required
- Reliable personal transportation required
- Experience with EPIC charting system preferred
- Two professional references covering 1 year within the last 3 years
