Job Opportunity – Occupational Therapist – Portland, OR 97225

Start Date: September 21, 2026

Location: West Portland, OR and surrounding areas

Schedule: Day shift, 8-hour shifts (08:00 - 16:30), 5 days per week. Up to 6 patient visits per day with 30 productivity points standard. No weekend requirement. No on-call requirement.

Highlights:

• Home health occupational therapy position serving patients in their homes with 1:1 care
• 91-day assignment with potential for extension
• Free parking and mileage reimbursement at IRS standard rate
• Efficient scheduling minimizes drive time; occasional maximum radius of approximately 60 miles per day
• Business casual or scrubs acceptable; flexible dress code
• Work with diverse patient populations including post-stroke, orthopedic post-op, Parkinson's, ALS, MS, and dementia patients

Requirements:

• Active Oregon Occupational Therapy license
• Minimum 1 year of skilled home health experience required
• OASIS documentation experience required
• Basic Life Support (BLS) certification
• Reliable personal transportation required
• Experience with EPIC Remote Client for Home Health and Rover phone app preferred
• COVID-19 primary series vaccination required
• 2 professional references covering 1 year within the last 3 years
