Job Opportunity – Physical Therapist, Home Health – Portland, OR 97225

Start Date: September 21, 2026

Location: West Portland, OR and surrounding areas

Schedule: Day shift, 8-hour shifts (08:00 - 16:30), 5 days per week. Typically 1:1 patient visits with up to 6 patients per day. 30 productivity points standard. No weekend requirement. No on-call requirement.

Requirements:

• Active Oregon Physical Therapist license required
• Minimum 1 year of skilled home health experience required
• OASIS documentation experience required
• Basic Life Support (BLS) certification required
• EPIC charting experience preferred (facility uses EPIC Remote Client for Home Health and Rover phone app)
• Reliable personal transportation required
• COVID-19 primary series vaccination required
• Two professional references covering 1 year within the last 3 years
