Job Opportunity – Occupational Therapist – Seaside, OR 97138

Start Date: September 28, 2026

Location: Seaside, OR and surrounding coastal areas

Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30), Monday through Friday. Patient visits average up to 6 per day with 30 productivity points standard. 1:1 patient ratio. No weekend or on-call requirements.

Requirements:

  • Active Oregon Occupational Therapy license required at time of submission
  • NBCOT certification and current BLS (BCLS) certification required
  • Minimum 1 year of skilled home health experience required
  • OASIS experience required – must be proficient in OASIS documentation
  • Experience with EPIC Remote Client for Home Health and Rover phone app preferred
  • Reliable personal transportation required for home visits (occasional travel radius up to 60 miles)
  • COVID-19 primary vaccination series required
  • Two professional references covering 1 year within the last 3 years
