Description

Job Opportunity – Physical Therapist, Home Health – Seaside, OR 97138

Start Date: October 19, 2026

Location: Seaside, Oregon and surrounding coastal communities

Schedule: Day shift, 8-hour shifts (08:00 - 16:30), Monday through Friday. Up to 6 patient visits per day with a standard productivity expectation of 30 points. No weekend coverage required. No on-call responsibilities.

  • Well-organized scheduling minimizes drive time; occasional travel up to 60 miles with 60-90 minute total drive time per day
  • Mileage reimbursement provided at IRS standard rate with free parking
  • EPIC Remote Client for Home Health charting on laptop with Rover phone app and OASIS documentation
  • Business casual or scrubs allowed; flexible, professional dress code
  • 91-day assignment with guaranteed hours (facility may call off up to one shift per two-week pay period)

Requirements:

  • Valid Oregon Physical Therapy license or compact license (must be in hand at time of submission)
  • Minimum 1 year of skilled home health experience required
  • Basic Life Support (BLS) certification
  • COVID-19 primary vaccination series plus one booster required
  • OASIS documentation experience required
  • Reliable personal transportation required
  • Two professional references covering 1 year within the last 3 years
  • Experience with diverse patient populations including post-operative care, neurological conditions, orthopedic rehabilitation, and chronic disease management
