Background and Compliance Requirements

• Background check within 45 days of start, covering all locations lived/worked in past 7 years
• Education verification required for all candidates
• State-specific searches and registry checks as applicable (e.g., Washington state requirements if applicable)
• Drug screening: 10-panel drug screen within 90 days of start (no instant tests)
• Physical exam within 1 year of start, signed by MD, NP, PA, or DO
• Immunizations and health documents required:

  • TDAP (within 10 years or declination accepted)
  • Negative TB test or chest X-ray within 12 months
  • MMR (Measles, Mumps, Rubella) vaccination or positive titer
  • Hepatitis B vaccination or signed declination
  • Varicella vaccination or positive titer
  • Flu vaccine during flu season or declination
  • COVID vaccine or declination
• Fit mask testing required for travel candidates (except Medical Lab Scientists and Sterile Processing Techs)

Training and Competency Requirements

• Completion of Providence Health and Services WorkRamp modules prior to start, including:

  • Core modules for profession
  • Specialty-specific modules if applicable
  • Annual regulatory modules
• Competency exams as applicable (e.g., medication test/pharmacology competency)
• Epic training modules required for charting system access

Documentation and History

• Employment application covering at least last 5 years (7 years for APN/Subcontractors)
• Minimum of 2 references covering 1 year of employment in a comparable specialty/setting (6 months for CNAs)
• Skills checklists updated annually as applicable

Additional Notes

• Guaranteed hours: Facility may call off once per two-week pay period
• Floating policy acknowledgment required
• Badge photo required prior to start
• Respiratory safety clearance required annually for travel candidates
• Some state-specific and facility-specific requirements may apply depending on placement location
