Job Summary: Laboratory Travel/Contract Position

Location: Napa, California
Contract Length: 13 weeks

  • CPI (Crisis Prevention Institute) (if applicable)
  • State License (copy required; CPDH Certificate required for RTs in CA)
  • NURSYS Verification (for RNs)
  • Tech Certification Verification (for Allied Professionals)

Required Background Checks and Verifications

  • Background Check within 45 days of start, covering all locations lived/worked in past 7 years
  • EPLS/GSA/SAM Check (updated monthly)
  • HHS/OIG Check (updated monthly)
  • Education Verification (3rd party verification required for all candidates)
  • State-specific searches as applicable (e.g., Washington WSP/WATCH, OBRA Registry for CNAs)
  • Transcript/Diploma (required by Aya for education verification, especially for international education)

Health and Immunization Requirements

  • Physical Exam within 1 year of start, signed by MD, NP, PA, or DO
  • Fit Mask Test (annual, travel only)
  • Tetanus/Diphtheria/Tdap (within 10 years; declination accepted)
  • Negative TB or Chest X-Ray (within 12 months; annual updates required)
  • MMR Vaccines or Positive Titers for Rubeola, Mumps, Rubella
  • Hepatitis B vaccination or signed declination
  • Varicella vaccination or positive titer
  • 10 Panel Drug Screen within 90 days of start (no instant tests)
  • Flu Vaccine during flu season or declination
  • COVID Vaccine or Declination

Competency and Skills Requirements

  • Medication Test/Pharmacology Competency Exam (as applicable)
  • Color Blind Testing (if required by unit)
  • Competency Exams as applicable by profession/specialty
  • Skills Checklist (updated annually, required for applicable units)
  • Medication Aide Exam for CNAs in LTC specialty at Oregon Assisted Living facilities

Employment History and References

  • Employment Application covering at least last 5 years (7 years for APN/Subcontractors)
  • Minimum of 2 references covering 1 year of employment within last 3 years in comparable specialty/setting (6 months for CNAs)
  • References must be from supervisors; charge RNs acceptable if direct supervisors

Facility Specific Requirements

  • Floating Policy Acknowledgement required (may float to other Providence facilities nearby)
  • Badge Photo (passport style with neutral background) required
  • Completion of Providence WorkRamp Modules (Core, Specialty, Regional, and Annual Regulatory) as assigned based on profession and region
  • Epic Access Training Modules required for clinical staff
  • Providence System TB Questionnaire (effective for new placements starting 2/5/2024)
