Job Description

CVH Care Home Health is seeking a qualified Physical Therapy Assistant with at least 1 year of professional experience. Home health experience is preferred.

Assigned Territory

• Area #1: Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City
• Plus either Area #2: Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont
• OR Area #3: San Ramon, Dublin, Pleasanton, Sunol, Livermore

Work Options

• Guaranteed Hours Option:

  • Minimum 36 hours guaranteed per week
  • Must complete at least 30 points per week (points based on visits)
  • Willingness to travel outside the area or work weekends if needed
  • Guarantee does not apply during first/last week, holidays, or approved time off
  • Refusing patients voids guarantee

• Non-Guaranteed Hours Option:

  • No guaranteed hours
  • Weekend coverage may be required
  • May need to travel outside service area occasionally
  • Can refuse patients
  • Must be willing to see COVID+ and COVID PUI patients

Visit and Hours Calculation

• Routine Visit = 1 point = 1.33 hours
• Hours approved based on visits, covering drive/travel, visit, documentation, and patient/doctor communication time

Required Credentials and Compliance

• Current California PTA License
• BLS Certification
• Valid Driver’s License, Auto Insurance, and Auto Registration
• Up-to-date immunizations and screenings: TB (annual), MMR, Varicella, Hepatitis B, TDap (within 10 years), Flu (annual)
• Drug screen and physical exam

Important Notes for Agencies and Travelers

• Agency contracts must exactly match the confirmed assignment dates and terms
• All requested time off (RTO) must be disclosed at submission; no post-confirmation additions allowed
• Failure to comply with contract and time off policies may result in removal from the vendor list
