Location: White River Junction, VT (also serving Lebanon, NH; Bradford, VT; Windsor, VT)
Facility: VNH Vermont & New Hampshire
Employment Type: Contract (13 weeks)

Job Responsibilities

• Screen, evaluate, re-evaluate, and treat patients in a home health setting.
• Develop and implement appropriate treatment plans based on patient needs.
• Communicate and coordinate treatment activities with interdisciplinary team members and patients.
• Document evaluations, treatment goals, and progress.
• Educate patients and family members on treatment rationale and procedures.

Minimum Qualifications

• Bachelor of Science in Physical Therapy.
• Licensed Physical Therapist in Vermont and New Hampshire, or hold an unencumbered compact license.
• Minimum of one year experience as a Physical Therapist in a home healthcare setting.
