**Join Our Team as an RN Case Manager!** **Job Description:** - Exciting opportunity to make a real impact in patient care! - **Position:** RN Case Manager - **Shift:** Day 5x8-Hour (08:00 - 17:00) - **Experience Required:** 3 years of inpatient case management - **License:** Active California RN license - **Charting System:** Epic experience essential - Engage in **Care Coordination** and **Discharge Planning** - Collaborate with an interdisciplinary team including Social Services and Transportation - Maintain patient ratios of 1:15 during days and 1:40 on weekends - Handle an average daily census of 255 patients - Conduct approximately 15 patient visits per day - Utilize InterQual Criteria and Milliman Guidelines for patient assessments - Ensure compliance with CMS, HIPAA, and The Joint Commission standards - Work with various patient age groups: Adolescents, Adults, and Geriatrics - Collaborate effectively in **Utilization Review** practices - Orientation consists of 16 hours to ensure you are set up for success - Weekend Coverage: Every other weekend with no weekend commitment required **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
