Hospice Registered Nurse (RN) - Contract

Location: Grand Junction, Colorado

Job Description

Provide compassionate hospice nursing care, supporting patients and families with strength and courage.
Serve as a primary RN with an assigned caseload, leading the interdisciplinary team for those patients.
Deliver care in various settings including private homes, nursing homes, group homes, and assisted living facilities.
Act as a teacher to patients and caregivers, helping identify personal goals of care and building caregiver confidence.
Manage symptom control and provide generalist care across all diseases, requiring strong assessment and critical thinking skills.
Work a weekday schedule of 4-9 hour shifts, with occasional weekend backup and on-call duties.
Flexible daily scheduling to support work/life balance.
