**Join Our Team as a Case Manager and Explore the Beauty of Eureka!** **Job Description:** - **Position:** Case Manager (Registered Nurse) - **Experience Required:** Minimum of 2 years in acute Hospital Case Management, focusing on Care Coordination and discharge planning - **Travel Experience:** Required (1st time travelers will be considered with manager approval) - **Shift:** Day shift, 5x8 Hours (08:00 - 16:30) **Essential Responsibilities:** - Perform comprehensive Care Coordination and Discharge Planning - Utilize your knowledge of Admission Criteria, Appeals and Denials, and Benefits Eligibility - Engage in effective Care Coordination and Needs Assessment including DME orders - Process Prior Authorizations, with a strong emphasis on regulatory understanding, including CMS and HIPAA guidelines - Experience with HMOs, IPAs and familiarity with advance directives and POLST is required - Collaborate with multidisciplinary teams including Social Services **Work Settings:** - Acute Hospital - Case Management/Utilization Review - Float required to meet department needs - Weekend coverage required every other weekend - Patient ratios during days: 1:20 **Patient Demographics:** - Adolescents, Adults, Pediatrics, Geriatrics - Daily census: caseload up to 25 **Additional Perks:** - Free parking on the first day with assigned parking during orientation - Support for housing assistance is available for travelers - Explore the great outdoors — Easy access to 7 different state and national parks! **Note:** Candidates must possess current acute care experience within the last year.
