Behavioral Health Social Worker - Contract Opportunity

Location: Mammoth Lakes, California
Employment Type: Contract
Contract Length: 13 weeks

Job Description

The Hospital Social Worker collaborates with Case Manager RNs to develop post-acute care plans for patients facing personal, social, or environmental barriers to wellness and recovery. Responsibilities include:

• Intervening with patients who have complex psychosocial needs
• Assisting with eligibility determination for social programs and community assistance
• Providing crisis intervention to patients and families
• Coordinating and facilitating discharge plans for high-risk patients
• Communicating with patients and families to develop continuing care plans
• Assisting with resolving personal and environmental difficulties affecting health outcomes

Key Duties

• Support planning and post-discharge care
• Screen, assess, and provide resources related to social determinants of health
• Act as an expert on community social resources
• Refer patients to Behavioral Health for assessments or outpatient services
• Provide education and support for food access and housing
• Coordinate patient transportation programs
• Assist families in crisis to understand medical and lifestyle impacts
• Conduct financial assessments and referrals
• Arrange in-home care and equipment

Qualifications

• Bachelor’s degree in Social Work required; Master’s degree preferred
• California Licensed Clinical Social Worker (LCSW) required
• 3 to 5 years hospital social work experience preferred (use 5 years as the higher requirement)

Work Schedule

• Full-time, day shifts
