Health & Immunization Requirements

Physical exam within 12 months of start
Fit mask test within 1 year of start, renewed annually
Adult Tetanus/Diphtheria/Tdap vaccine within 10 years
Negative TB test (2-step PPD, QGOLD, or TSPOT) within 1 year
TB questionnaire if positive TB test, renewed annually
Measles (Rubeola), Mumps, Rubella: Positive titer or 2 vaccines required; boosters required if negative/equivocal
Hepatitis B: Positive titer, series, or declination accepted
Varicella: Positive titer, 2 vaccines, or physician-documented history required; boosters if negative/equivocal
Negative 10-panel drug screen within 90 days of start
Seasonal Flu vaccination or approved medical/religious exemption required during flu season (Oct 1 - Mar 31)
COVID-19 vaccination or approved medical/religious exemption required (dose 1 and fully vaccinated)

Competency Requirements

Medication Test/Pharmacology Competency Exam (80% passing or meet Angoff score)
Color Blind Testing (online acceptable)
Competency Exam (minimum 80% passing or meet Angoff score)
Unit-specific Skills Checklist required annually
