Job Opportunity: Outpatient Occupational Therapist (OT) - Travel/Contract

Required Certifications & Licenses

Compliance & Documentation Requirements

• Background Check: Must be completed within 45 days of start, covering all locations lived/worked in past 7 years. Includes SSN trace, death master index, nationwide checks, education verification, and more.
• Education Verification: Required for all candidates.
• References: Minimum 2 supervisor references covering 1 year of employment within the last 3 years in a comparable specialty/setting.
• Employment Application: Must include at least last 5 years of employment with month/year format.
• Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO, indicating fitness to work.
• Drug Testing: 10-panel drug screen within 90 days of start (no instant tests).
• Immunizations & Health Documents:

  • TDAP (within 10 years or declination)
  • Negative TB test or chest X-ray within 12 months
  • MMR (2 vaccines or positive titer)
  • Hepatitis B vaccination or declination
  • Varicella (2 vaccines or positive titer)
  • Flu vaccine during flu season or declination
  • COVID vaccine or declination
• Respiratory Safety Clearance: Required annually for travel candidates.
• Fit Mask Testing: Required for travel candidates, updated annually.

Training & Competency Requirements

• WorkRamp Modules: Completion of required Core and Specialty-specific modules prior to start, including annual regulatory requirements. Modules assigned based on region, specialty, and facility.
• Epic Training: Completion of Epic modules required for charting system access.
• Competency Exams: Medication test/pharmacology competency and other specialty-specific exams as applicable.

Additional Notes

• Floating Policy: Acknowledgement required; may float to other Providence facilities within reasonable distance.
• Scheduling: Block scheduling may be accommodated at manager discretion.
• Locals: Local candidates are accepted.
• Pending Licenses: Not accepted; must have active CA OT license at time of submission.
