Experience Requirements

• State Professional License (CPDH Certificate required for RTs in CA)
• NURSYS Verification for RNs (if applicable)
• IV Certification (for LVN/LPN/Other, if applicable)
• Management of Assaultive Behavior (MAB) (if applicable)
• NIH Stroke Certification (required every 2 years if applicable)
• CPI (Crisis Prevention Institute) certification (if applicable)
• Other miscellaneous certifications as required by clinical track or facility

Background and Compliance Requirements

• Background check within 45 days of start, covering all locations lived/worked in past 7 years
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start, updated monthly
• Education Verification required for all candidates (nursing degree verification for RNs/LVNs)
• State-specific searches (Washington and Oregon) if applicable
• Drug testing: 10 panel drug screen within 90 days of start (no instant tests)
• References: Minimum 2 supervisor references covering 1 year employment within last 3 years (6 months for CNAs)
• Employment application covering at least last 5 years (7 years for APN/Subcontractors)
• Skills checklists as applicable, updated annually

Health and Safety Requirements

• Physical exam within 1 year of start, signed by MD, NP, PA, or DO
• Fit mask test (N95 or equivalent) required for travel candidates, updated annually
• Immunizations/vaccinations:

  • TDAP (Tetanus/Diphtheria/Pertussis) within 10 years or declination accepted
  • Negative TB test or chest X-ray within 12 months, updated annually
  • MMR (Measles, Mumps, Rubella) vaccination or positive titer, declination accepted
  • Hepatitis B vaccination or signed declination (special rules for Alaska placements)
  • Varicella vaccination or positive titer, declination accepted
  • Seasonal flu vaccine during flu season or declination accepted
  • COVID vaccine or declination required
• Respiratory safety clearance required annually for travel candidates

Training and Competency Requirements

• Medication Test/Pharmacology Competency Exam as applicable
• Competency exams as required by specialty
• Color Blind Testing if required by unit
• Specialty-specific competency exams (e.g., Medication Aide exam for CNAs in LTC Oregon facilities)
• Providence WorkRamp modules:

  • Core regulatory modules annually
  • Specialty-specific modules for ER, L&D, leadership roles, and others as applicable
  • Regional and facility-specific modules depending on assignment location
• Epic training modules required for certain regions and roles

Additional Notes

• Float policy acknowledgment required; may be asked to float to other Providence facilities nearby
• Passport-style badge photo required for badge issuance
• Radiation badge issued by facility if applicable
• Some state-specific attestations and forms may be required for multi-state licensed nurses

This position requires compliance with extensive background checks, certifications, health screenings, and training modules to meet Providence Health and Services standards. Candidates should be prepared for weekend shifts and a 13-week travel contract in Santa Rosa, CA.
