Experience & Licensure Requirements

• Years of Experience Required: Minimum 2 years of Pediatric OT experience (hard stop)
• Prior Travel Experience: Not required
• Licenses & Certifications:

  • Washington OT License (pending license accepted)
  • NBCOT Certification
  • BLS Certification

Required Certifications (Must be current and valid)

• BLS (Basic Life Support)
• NBCOT Certification
• Washington OT License
• Additional certifications may be required based on clinical track (verify with clinical leadership)

Compliance & Documentation Requirements

• Background check within 45 days of start, covering all locations lived/worked in past 7 years
• Education verification required
• State-specific background checks and registry searches as applicable (Washington state requirements)
• Physical exam within 1 year of start, signed by MD, NP, PA, or DO
• Immunizations and health screenings including:

  • TB test or chest X-ray (annual update required)
  • TDAP vaccine within 10 years
  • MMR (Measles, Mumps, Rubella) vaccination or positive titer
  • Hepatitis B vaccination or signed declination
  • Varicella vaccination or positive titer
  • Flu vaccine during flu season or signed declination
  • COVID vaccine or signed declination
• Drug screening: 10-panel drug screen within 90 days of start (no instant tests)
• Respirator clearance (RespSafety) required annually for travel candidates
• Fit mask testing required (annual update)

Training & Competency Requirements

• Completion of Providence Health and Services WorkRamp modules prior to start, including:

  • Annual regulatory requirements
  • Specialty-specific modules for Pediatric OT as applicable
  • Facility-specific modules (e.g., Basics of Trauma Informed Care)
• Medication and pharmacology competency exams as applicable
• Skills checklists updated annually as required

Additional Notes

• Candidates must submit a photo for badge issuance (passport style, neutral background)
• References: Minimum 2 supervisor references covering at least 1 year of employment in a comparable pediatric specialty within the last 3 years
• Employment application covering at least the last 5 years required
• No floating between units expected, but float policy acknowledgment required in case of regional needs
• Pending licenses accepted, but all required documentation must be submitted at time of application
