Job Description

• Provide individualized physical therapy programs including assessment, treatment, planning, implementation, education, and communication to maximize patient progress toward functional goals.
• Maintain positive working relationships with other health professionals and ancillary staff.
• Adapt to a collaborative "Friends healing Friends" approach within the community.
• Treat a variety of patient needs across settings.
• Caseload is approximately 85% outpatient and 20% long-term care (LTC) and skilled swing bed patients.
