Work Schedule & Requirements

- Floating: No floating between units required
- On-call: No
- Block Schedule: Open to Sat-Mon block schedule
- Guaranteed Hours: Facility may call off 1 shift per 2 weeks
- Charting System: Epic
- Scrub Color: Radiology staff wear dark solid colors
- Parking: Free parking on first day; assignment parking provided during orientation

Compliance & Documentation Requirements
Background Checks
- Background check within 45 days of start, covering all locations lived/worked in past 7 years
- Must include SSN Trace, Death Master Index, Counties, Nationwide, National Sex Offender, FACIS Level 3, Education Verification, MVR (if Home Health), and others as applicable
- Background checks updated annually
- Special state-specific searches for Washington and Oregon if applicable
Health Documents
- Physical Exam within 1 year of start (signed by MD, NP, PA, or DO)
- Fit Mask test (annual, onsite if no recent record)
- Tetanus/Diphtheria/Tdap vaccine within 10 years (declination accepted)
- Negative TB or Chest X-Ray within 12 months (annual update required)
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- 10 Panel Drug Screen within 90 days of start (no instant tests)
- Flu vaccine during flu season or declination
- COVID vaccine or declination
Competencies & Skills
- Medication Test/Pharmacology Competency Exam (as applicable)
- Competency Exams as required by specialty
- Color Blind Testing (if required by unit)
- Skills Checklist updated annually (if applicable)
References & Employment History
- Minimum 2 references covering 1 year of employment within last 3 years in comparable specialty/setting (6 months for CNAs)
- Employment application covering at least last 5 years (7 years for APN/Subcontractors) with month/year format and explanation of gaps
Training & Modules
- Completion of Providence WorkRamp Core and Specialty Modules as assigned based on profession and region
- Epic training modules required prior to moving to PDW status
- Annual regulatory and specialty-specific training modules required
- Modules assigned via Aya's LMS platform, WorkRamp

- Candidate must comply with Providence’s floating policy acknowledgment (may be required to float to other Providence facilities within reasonable distance)
