Work Schedule & Requirements

• Block Schedule: Open to Sat-Mon block schedule
• Guaranteed Hours: Facility may call off 1 shift per 2 weeks

Compliance & Documentation Requirements

Background Checks

• Background check within 45 days of start, covering all locations lived/worked in past 7 years
• Must include SSN Trace, Death Master Index, Counties, Nationwide, National Sex Offender, FACIS Level 3, Education Verification, MVR (if Home Health), and others as applicable
• Background checks updated annually
• Special state-specific searches for Washington and Oregon if applicable

Health Documents

• Physical Exam within 1 year of start (signed by MD, NP, PA, or DO)
• Fit Mask test (annual, onsite if no recent record)
• Tetanus/Diphtheria/Tdap vaccine within 10 years (declination accepted)
• Negative TB or Chest X-Ray within 12 months (annual update required)
• MMR (Measles, Mumps, Rubella) vaccination or positive titer
• Hepatitis B vaccination or signed declination
• Varicella vaccination or positive titer
• 10 Panel Drug Screen within 90 days of start (no instant tests)
• Flu vaccine during flu season or declination
• COVID vaccine or declination

Competencies & Skills

• Medication Test/Pharmacology Competency Exam (as applicable)
• Competency Exams as required by specialty
• Color Blind Testing (if required by unit)
• Skills Checklist updated annually (if applicable)

References & Employment History

• Minimum 2 references covering 1 year of employment within last 3 years in comparable specialty/setting (6 months for CNAs)
• Employment application covering at least last 5 years (7 years for APN/Subcontractors) with month/year format and explanation of gaps

Training & Modules

• Completion of Providence WorkRamp Core and Specialty Modules as assigned based on profession and region
• Epic training modules required prior to moving to PDW status
• Annual regulatory and specialty-specific training modules required
• Modules assigned via Aya's LMS platform, WorkRamp

• Candidate must comply with Providence’s floating policy acknowledgment (may be required to float to other Providence facilities within reasonable distance)
