Job Details

Required Certifications

• State License (copy of professional license required; CPDH certificate required for RTs in CA)
• ACLS (Advanced Cardiac Life Support) - AHA or Canadian Heart & Stroke Foundation certification
• BLS (Basic Life Support) - AHA or Canadian Heart & Stroke Foundation certification
• PALS (Pediatric Advanced Life Support) - AHA or Canadian Heart & Stroke Foundation certification
• NRP (Neonatal Resuscitation Procedure) - Current certification by AAP, AHA, or Canadian Paediatric Society
• Other certifications as applicable per clinical track and specialty requirements (e.g., Chemotherapy, EFM, IV Certification, MAB, NIH Stroke Certification, CPI)

Required Background Checks and Verifications

• Background Check within 45 days of start, covering all locations lived/worked in past 7 years
• EPLS/GSA/SAM and HHS/OIG checks within 30 days of start, updated monthly
• Education Verification (3rd party) for all candidates
• State-specific searches as applicable (e.g., Washington State Patrol Search for WA placements)
• NURSYS Verification for RNs
• Drug Testing: 10 Panel Drug Screen within 90 days of start (no instant tests)
• References: Minimum 2 supervisor references covering 1 year of employment within last 3 years in comparable specialty/setting
• Employment Application: Must include at least last 5 years of employment with month/year format

Health and Immunization Requirements

• Physical Exam within 1 year of start, signed by MD, NP, PA, or DO
• Fit Mask test (annual, travel only)
• Tetanus/Diphtheria/Tdap vaccine within 10 years or declination accepted
• Negative TB test or Chest X-Ray within 12 months, updated annually
• MMR (Measles, Mumps, Rubella) - 2 vaccines or positive titer or declination
• Hepatitis B vaccination or signed declination (special rules for Alaska placements)
• Varicella - 2 vaccines or positive titer or declination
• Flu Vaccine during flu season or declination
• COVID vaccine or declination
• Respiratory Safety Clearance (annual for travel candidates)

Competency Requirements

• Medication Test/Pharmacology Competency Exam as applicable
• Competency Exams as required by specialty
• Color Blind Testing if required by unit
• Specialty-specific competency exams (e.g., Medication Aide exam for CNAs in LTC Oregon facilities)

Facility Specific Requirements

• Completion of Providence WorkRamp Core and Specialty Modules prior to start (assigned by Clinical Staffing)
• Photo for badge (passport style with neutral background)
• Floating Policy Acknowledgement (may be required to float to other Providence facilities within reasonable distance)
• Epic Access training modules as applicable (especially for Per Diem in SoCal, OR, Spokane)
• System TB Questionnaire (effective for new placements starting 2/5/2024)

Additional Notes

• Candidates with gaps or overlaps in employment must provide clarification on application
• Background checks must be updated annually or as specified
• Returning candidates with less than 30-day gap may not require new background or drug screen if current at another Providence facility
• Some certifications and requirements may be waived for non-clinical or remote candidates
