Location: Hot Springs, South Dakota

Job Description

Provide physical therapy services to support the treatment of illness and/or injury through physical exercises and therapies aimed at rehabilitation and restoration of body functions. Deliver therapy programs for patients of all ages with developmental deficits, physical injury, or illness.

Key Responsibilities

• Coordinate and provide direct therapeutic care to assigned clients.
• Teach rehabilitation skills to clients and/or caregivers to promote maximum independent functionality and safety.
• Provide preventive and rehabilitative physical therapy procedures tailored to client needs and age.
• Develop and revise plans of care based on assessments, client history, diagnosis, physician orders, and client goals, including discharge planning.
• Perform or monitor treatments using physical agents (heat, water, electricity) and equipment such as whirlpool baths, electrical muscle stimulation, and ultrasound.
• Follow utilization guidelines and protocols.
• Communicate significant clinical findings and changes to physicians and involved personnel.
• Document all skilled and medically necessary care thoroughly and timely, ensuring compliance with Medicare, State, Federal, JCAHO, insurance, and payer requirements.
• Maintain client and organizational confidentiality.
• Participate in Quality Management programs and complete required in-service training.
• Conduct supervisory visits and prepare/update written instructions for Physical Therapy Assistants and Home Health Aides as applicable.
• Supervise Physical Therapy Assistants and Home Health Aides in accordance with laws and agency policies.
• Participate in staff meetings, case conferences, business development, and customer service programs.
• Demonstrate leadership through communication, relationship building, and teamwork.

Required Qualifications

• Licensed Physical Therapist (PT) in South Dakota.
• Experience in physical therapy practice consistent with job responsibilities.
• Ability to develop and implement individualized treatment plans.
• Strong documentation and communication skills.
