Job Summary

The Care Manager (CM) II collaborates with patients, families, physicians, and multidisciplinary teams to ensure smooth patient progression through the continuum of care from admission to discharge. Responsibilities include case management, care coordination, discharge planning, and optimizing patient flow to enhance continuity, safety, satisfaction, and length of stay management.

Key Responsibilities

• Coordinate case management functions within patient care and discharge planning processes.
• Facilitate efficient and cost-effective patient care progression throughout the continuum of care.
• Serve as a patient advocate regarding treatment decisions and end-of-life issues.
• Monitor patient length of stay and collaborate with interdisciplinary teams to remove barriers and expedite discharge.
• Implement and monitor patient care plans for effectiveness and appropriateness.
• Identify and resolve delays or obstacles to discharge using advanced conflict resolution skills.
• Collaborate with medical, nursing, and ancillary staff to eliminate barriers to care delivery.
• Conduct patient/family interviews to assess social, emotional, and financial factors impacting health status.
• Assess discharge planning needs and recommend post-acute care options (e.g., rehabilitation, skilled nursing, home health).
• Initiate discharge planning at admission and coordinate post-hospital service referrals.
• Negotiate and coordinate resources with payors, agencies, and vendors.
• Ensure communication and documentation of care plans to patients, families, and healthcare teams.
• Provide culturally sensitive interventions and education tailored to patient backgrounds.
• Participate in clinical performance improvement activities related to length of stay, resource utilization, and readmissions.
• Actively engage in multidisciplinary rounds and escalate cases as appropriate.
• May assist with special projects and serve as a preceptor or orienter to new associates.

Required Qualifications

• Valid RN license in Texas
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Minimum 2 years of case management experience, including experience within the last year
• Experience in utilization management and utilization review (InterQual)
• Experience with MCG criteria and InterQual criteria
• Acute hospital case management experience
• Experience with Epic electronic health record system

Work Schedule & Environment

• 5 days per week, 8 hours per day.
• May be asked to arrive at 7:30 AM and stay until all cases are completed.
• Work involves moving around the hospital for the majority of the day with some office time.
• Fast-paced and sometimes stressful environment requiring multitasking and sound judgment.

• Prior travel experience is not explicitly required for this position.
• This is a travel assignment with a potential temp-to-perm option.
