**Join a Dynamic Team at Helen Porter Rehab & Nursing Center!** **Job Description:** - Join our dedicated Long-Term Care team at a facility with a total of 98 beds! - Work in a supportive environment with a focus on patient care and teamwork. - Schedule: - Rotating shifts of 4 x 8s one week and 5 x 8s the next. - Minimum of every other weekend required. - Floating may be necessary to meet unit and patient care needs. - Unique Opportunity: - Engage in dementia-capable care (certification preferred). - Work collaboratively with a nursing team ratio of 1:20. - Patient Population: - Focus on a diverse group of patients aged 60 and older in long-term care. - Documentation: - Utilize EPIC for documentation and medication management. - Additional Information: - No public transportation available; reliable personal or rental vehicle strongly recommended. - Ensure your vehicle is equipped with all-season or snow tires from November to April. - All employment gaps over 90 days must be explained. - Complete work history information required prior to submission. - Orientation: - Bi-weekly orientations every Monday to help you hit the ground running! **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
