Physical Therapist - Contract Opportunity

Location: Marysville, California

Job Summary

Seeking a compassionate and skilled Physical Therapist (PT) to join an interdisciplinary team in a Skilled Nursing Facility (SNF). The PT will evaluate residents' physical conditions, develop personalized treatment plans, and deliver rehabilitative care to improve mobility, restore function, and help patients achieve the highest possible level of independence.

Key Responsibilities

• Perform thorough assessments of patients' physical abilities, movement limitations, and pain levels.
• Develop and implement individualized treatment plans based on evaluations, medical history, and patient goals.
• Deliver evidence-based physical therapy interventions to improve strength, coordination, balance, and mobility.
• Collaborate with physicians, nursing staff, occupational and speech therapists, and family members to coordinate patient care.
• Monitor patient progress, adjust therapy plans as needed, and document all care in compliance with state and federal guidelines.
• Educate patients and caregivers on safe mobility techniques, home exercises, and equipment use.
• Ensure compliance with regulatory requirements including Medicare/Medicaid documentation and facility policies.
• Participate in care planning meetings and interdisciplinary team conferences.
• Supervise physical therapist assistants (PTAs) and aides as needed.
• Maintain a safe and clean work environment.

Required Qualifications

• Graduate of an accredited Physical Therapy program.
• Current California state license to practice as a Physical Therapist (PT).
• CPR certification (preferred or required depending on facility).
• Experience in a Skilled Nursing Facility or geriatric rehabilitation setting preferred.
• Excellent communication, documentation, and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.

Work Environment

• Skilled Nursing Facility (SNF) providing inpatient, long-term care, and short-term rehabilitation.
• Job may involve lifting, standing, walking, and assisting patients with transfers and mobility.
