Description

Job Opportunity: RN Hospice Case Manager

Location: Barre, Vermont
Employment Type: Contract
Contract Length: 20 weeks
Work Schedule: Monday - Friday, 36 hours per week

Job Description

Provide hospice care in a home setting as an experienced Hospice RN.
Manage hospice cases effectively.
Prior experience with rural travel is preferred.
Experience with McKesson/Netsmart EMR systems is preferred.
Preference for candidates with no Return to Office (RTO) requests for this contract.

Required Qualifications

Valid RN license in Vermont
Minimum of 1 year of case management experience, including within the last year
BLS certification
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Experience in a Home Health setting
Hospice nursing experience

Additional Information

This position involves travel to rural areas.
Work schedule is consistent Monday through Friday.
