Job Summary

Provide hospice case management focused on pain management and comfort for patients with terminal or chronic illness. Collaborate with interdisciplinary teams, patients, and families/caregivers to coordinate and communicate care needs.

Key Responsibilities

• Develop and periodically review patient care plans in collaboration with physicians, patients, and caregivers.
• Coordinate care plans through interdisciplinary communication and case conferences.
• Perform ongoing assessments of clinical, functional, and emotional status to identify issues or progress.
• Communicate significant changes to physicians and team members.
• Document evaluations, patient responses, and progress toward goals.
• Educate patients and caregivers on care techniques and end-of-life symptom management.

Required Qualifications

• Valid Registered Nurse (RN) license in Vermont
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Minimum of 2 years of hospice nursing experience, including experience within the last year
