Physical Therapist - Skilled Nursing Facility (SNF)
Location: Seattle, Washington
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Details
- Facility Type: Long-Term Care (LTC) / Skilled Nursing Facility (SNF)
- Shift: Day shift, 8 hours per day (8:00 AM - 4:30 PM), 5 days a week (40 hours/week)
- Schedule: No weekends required
- Ratios: 1 therapist to 15-25 patients
- Start Date: ASAP
- No on-call or floating between units
- Block schedules (e.g., 4x10s or 3x12s) are not accommodated
- Locals within 50-75 miles are accepted and considered local
- Pending license accepted
- Guaranteed hours with possibility of up to one shift off per pay period (biweekly)
- Own transportation required
- COVID-19: Primary vaccination series plus one booster required
- Attire: Black scrubs
- Charting system: Net Solutions
- Parking: Free
- Mileage reimbursement: IRS standard
Required Qualifications
- Valid Physical Therapist license in Washington (pending license accepted)
- Minimum 1 year of Physical Therapist experience within the last year
- Experience in a Long Term Care (LTC) or Skilled Nursing Facility (SNF) setting
- Basic Life Support (BLS) certification required
- Must have own transportation
- COVID-19 vaccination as noted above
Certifications & Competencies
- BLS (Basic Life Support) - AHA or Canadian Heart & Stroke Foundation certification
- Other certifications (e.g., ACLS, PALS, NRP) not required unless specified by clinical track
- Medication/Pharmacology competency exam as applicable
- Competency exams required as applicable by profession/specialty
Background & Compliance Requirements
- Government-issued photo ID (Driver’s License or State ID)
- Social Security Card (as applicable for I9 and state requirements)
- Background check within 90 days covering all locations lived/worked in past 7 years; must be updated annually
- Washington State Patrol (WSP/WATCH) search within 90 days of start, updated every 2 years for SNF/LTC facilities
- Education verification required
- Employment application covering last 5 years (7 years for APN/subcontractors)
- Minimum of 2 professional references covering 1 year of employment in a comparable specialty/setting within last 3 years
- Drug screen: 9-panel within 12 months of start (instant acceptable)
- Physical exam and immunizations as per facility policy (see Health Requirements)
Health & Immunization Requirements
- Physical exam within 1 year of start, updated annually
- Fit mask test (respirator clearance) required annually for travel staff
- Tetanus/Diphtheria/Tdap vaccine within 10 years (declination accepted)
- Negative TB test or chest X-ray within 12 months, updated annually
- MMR (Measles, Mumps, Rubella): 2 vaccines or positive titer; declination accepted
- Hepatitis B: vaccination series or signed declination
- Varicella: 2 vaccines or positive titer; declination accepted
- Seasonal flu vaccine during flu season (declination accepted)
- COVID-19 vaccination record preferred; declinations not required but unvaccinated must follow infection control policies
Additional Notes
- Facility requires completion of annual regulatory and specialty-specific training modules via LMS (Workramp)
- Photo for badge required (passport style, neutral background)
- Facility-specific background checks and attestations may be required prior to start
- No floating to other units expected, but float policy acknowledgment required
- References and employment history must be verifiable and meet facility standards
This position is ideal for experienced physical therapists with prior travel contract experience in skilled nursing or long-term care settings who meet Washington state licensure and immunization requirements.
