Primary RN - Case Manager (Community Hospice)
Location: Grand Junction, Colorado
Employment Type: Contract
Contract Length: 6 weeks
Job Description
- Provide compassionate hospice nursing care, supporting patients and families with strength and courage.
- Serve as a primary RN with an assigned caseload, leading the interdisciplinary team for those patients.
- Deliver care in various settings including private homes, nursing homes, group homes, and assisted living facilities.
- Act as a teacher to patients and caregivers, helping them identify personal goals of care and build confidence in caregiving.
- Manage symptom control and perform comprehensive assessments requiring critical thinking.
- Work schedule is Monday to Friday, 8 AM to 5 PM, with occasional weekend backup and on-call duties.
Required Qualifications
- Valid RN license in Colorado
- Minimum of 2 years of case management experience, including experience within the last year
- Current BLS certification
- Associate of Science in Nursing or Associate Degree in Nursing
- Hospice nursing experience
Additional Information
- This role involves community-based hospice care with a focus on personalized patient-centered service.
- Prior travel experience is not indicated as a requirement.
