Physical Therapist — Home Health
Location
- Albuquerque, New Mexico
- Home health visits within a 45-mile radius of the Albuquerque area
Contract
- 13 weeks
- Contract
- 30 hours per week
- Monday–Friday
Required Qualifications
- Valid Physical Therapist license in New Mexico
- BLS
- Driver's License
Experience
- Two years of clinical experience preferred
- Home health experience is a plus, but not required
Key Responsibilities
- Provide physical therapy services within the legal scope of practice
- Complete evaluations, progress notes, reassessments, recertifications, and other required documentation
- Develop and update individualized plans of care based on physician orders and patient needs
- Deliver skilled interventions and maintenance therapy programs
- Supervise Physical Therapist Assistants and Home Health Aides as allowed by state practice acts
- Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies
- Participate in interdisciplinary communication, quality improvement, staff education, and compliance activities
- Maintain timely, accurate clinical documentation and follow agency, Medicare, state, HIPAA, and privacy requirements
