Home Health & Hospice RN

Job Overview

Contract role in St. Johnsbury, Vermont
Day shift, Monday–Friday, with possible weekend shifts
36 hours per week
Contract length: 12 weeks

Role Summary

Provide professional nursing services to clients of all ages in a home health and hospice setting
Responsibilities include admission, evaluation, and care plan development
Work as part of a coordinated homecare team

Required Qualifications

Valid RN license in Vermont
Associate of Science in Nursing / Associate Degree in Nursing
BLS
Home Health (1 year of experience)
Home Health experience within the last year
Hospice experience
