Provides home health case management for patients, including screening, evaluation, treatment, care planning, coordination with an interdisciplinary team, patient/caregiver education, and documentation.

Required Qualifications

Valid RN license in New Hampshire
Associate of Science in Nursing / Associate Degree in Nursing
Home Health (2 years of experience)
Home Health experience within the last year
Experience in a Home Health setting
