Home Care Physical Therapist

Job Overview

Location: Barre, Vermont
Employment type: Contract
Contract length: 13 weeks
Schedule: Tuesday–Saturday, 8:00 AM–5:00 PM
Hours per week: 36
Setting: Home health / hospice
Patient care model: One-on-one therapy in patients’ homes

Required Qualifications

Valid Physical Therapist license in Vermont
1 year of Physical Therapist experience
Physical Therapist experience within the last year
Experience in a Home Health setting
Experience with OASIS

Preferred Qualifications

McKesson/Netsmart EMR experience

Key Responsibilities

Assess patient condition and develop individualized plans of care
Collaborate with the interdisciplinary team to meet patient needs
Perform standard therapy measurement techniques, including range of motion, strength, and balance tests
Use therapeutic modalities to maximize care provided to clients

Scheduling Note

No requested time off is preferred
