Physical Therapist Assistant — Home Health

Required Qualifications
- Valid Physical Therapy Assistant license in Colorado
- Driver's License
Responsibilities
- Provide skilled therapy services in patient homes
- Carry out established treatment plans involving therapeutic exercise, gait training, balance activities, and functional mobility intervention
- Monitor and communicate patient progress and changes to the supervising PT
- Encourage patient participation and reinforce goals
- Complete timely, accurate documentation in EMR
- Collaborate with therapists, nurses, caregivers, and other care team members
